Both a living will and a health care proxy are forms of Advance Directives. The living will details what you would like to have done for you in the event that you are unable to tell someone yourself. For example, "I would like to have CPR, or I would like to have a feeding tube." A health care proxy is someone that you name that will make these decisions for you in the event you are unable to make them yourself. Example: "I would like my mother to make my decisions." The health care proxy would only have the power to make decisions if you could not, for example if you were in a coma.
